Who is Integra Managed Care?
Integra Managed Care is a New York State Managed Long Term Care Plan designed for adults living with long-term disabilities. Our goal is to help our members live safely and independently in the comfort of their own home. At Integra, you will have a dedicated team of nurses and social workers who will ensure that your care and services help you live as independently as possible.

Through Integra Managed Care, you will have access to a wide array of services that can be tailored to meet your needs. As an Integra Managed Care member, you will have a dedicated Care Management Team that includes a Registered Nurse, Social Worker, and Coordinator who will work with you, your loved ones, and your health care providers to ensure all your long term care needs are met. We are committed to helping you stay healthy, safe, and living independently in the comfort of your own home.

Who Can Join Integra Managed Care?
Individuals who meet the following requirements may be eligible to join Integra Managed Care:
- At least 18 years of age
- Have or are eligible for New York State Medicaid
- Reside in Integra's service area (Bronx, Manhattan, Brooklyn, Queens, Staten Island, Nassau, Suffolk, or Westchester)
- In need of community-based long term care (for more than 120 days) based on an individualized medical assessment
- Capable of safely living in or returning to the community
